Usually the petition is no longer valid once it is withdrawn. It may be necessary to start the process all over.
OPT delay more than 90 days, finally received approval notice but not the card yet, can i start working with the notice?

No you cannot work with the notice. You must have your card to start working.

You should be able to change back to H-1B and you may still be cap exempt.

Once an RFE is issued it takes the application off the normal processing track. Yes it can take longer than 15 days.

The filing fees with the government can be a couple thousand dollars. If you hire an attorney it can cost several thousand more.

One should stay with there employer for at least six months after being approved for residency based on that employment. Longer is better though.
