OUTCOME: Client has birth certificate, Oregon ID and a Social Security Card all with same spelling of name.

I assisted an individual to obtain his birth certificate from another jurisdiction and his Oregon ID Card. Issues included correcting misspellings of the person's name with Social Security Administrati ... on and Oregon DMV. Tasks include completing an Affidavit of Identity for notarization, Freedom of Information request, telephone and and written advocacy. We prepared for multi-state litigation: Administrative hearing in Oregon, Circuit Court name-change procedures and a possible suit against a state. Prior to filing, we were granted the birth record and thus we were able to obtain Oregon ID.
