On appeal, appellate court agreed with me that the typed name does satisfy the subscription requirement thereby prevailing on our claim against opponent who tried to escape his obligations under the contract.

Held: Blood test result document is an affidavit as a matter of law, and without the required signature and sworn oath, it was declared, on appeal, to be inadmissible, restoring my client's driver's license when no one thought it possible. I did, and we won.
