the form is sent to you by the government after the approval of 212(e) waiver requirement. I assume you have a J-1 visa with a 2 year residency...

If you ask for it. It is your file. The attorney can charge you for making the copies though.

Conditional green card gives you the same rights as a permanent green card. Just 90 days prior to the expiration you need to make sure and file...
