It will depend on the laws of the country you are in. Philippines is a Catholic country, so you may have issues.

You are still a citizen of Philippines. I would recommend also contacting the local consulate to see if they can change the name.

You can file for a change of status from a H4 to a H1 and upon approval, should be able to work.

It is not relevant to a H1 but you should have it corrected to avoid issues when you file a green card.
