It seems unlikely that you would be able to get the ticket dismissed due to the mistake about race. However, it does raise the question about the officer's perceptions and that may go to the more important issue of whether there was a traffic infraction. You may want to review the details with an attorney to see if you have any possible defenses.

His rights are not affected and do not depend on what happened with your charges. If he has already plead guilty and was convicted, he will not be able to "take back" the deal. If he simply agreed to take the deal but has yet to enter the plea, he needs to speak with his attorney about his options.

Probation transfers from one Oregon county to another are fairly routine. However, it is likely that you would remain on probation in Clackamas County and Polk County would provide the supervision services. The process usually involves asking your current probation officer and explaining the reasons for the request. If a Polk County PO is already looking into your proposal, the process seems to have begun. If you want to know what other steps, if any, are necessary you should ask your current PO.
I am an Oregon attorney. You have 30 days after entry of the judgment of conviction to initiate your appeal. However, as detailed in your plea petition, which should have been reviewed and signed by you, your rights of appeal are limited to the sentence and not the conviction. You must be able to show that the sentence exceeds the maximum allowed by law or is unconstitutionally cruel and unusual.
The sentence is going to be determined by a number of factors: criminal history, seriousness of the second offense, your completion of any probation requirements at the time of the hearing, and the reason for your failure to appear. You certainly violated bench probation by committing another crime while on probation so your probation could be revoked or it could be converted to formal probation and extended. The most likely sentence will depend on those details not present in your question.
