Statement of use.

The actual Statement of Use is an electronic form at the USPTO TEAS site. The type of specimen depends on whether the application covers goods or services. When submitting the Statement of Use electronically, photographs showing boxes containing the goods with the mark printed thereon, the mark printed on the good itself, a label, hangtag, etc. Specimens for services include advertisements, brochures, websites, all advertising the services.
