She suffered a fall in the nursing home, after which she was on a 72-hour fall watch which required the nursing staff to make a full assessment of her condition each shift. The nursing staff failed to recognize signs and symptoms of progressive ischemia from a blood clot while she was on the fall watch, leading to the amputation of her leg.
