    It sounds like you may have a workers compensation claim. If you haven't already, you should let your supervisor know what happened so s/he can complete an incident report. You should also try to figure out whether there is a security camera or surveillance of the area where this happened, and make sure the video gets saved. Unless you believe some third party was responsible for what happened, you will likely be limited to filing a workers comp claim.

If you end up settling with the insurance company, make sure it takes into account ALL the expenses you have had or may have if you haven't had all the dental work done at this point.     In Wisconsin the dog can be put down especially after multiple bites. The best thing to do is maintain communication with the local law enforcement office to be clear on the rules in your area.

    The statute of limitations for negligence in Wisconsin is three years. The time limit can be extended if the plaintiff was a minor at the time of the accident.

    You should have a local personal injury attorney take a look at the accident report. Remember that the police are not generally concerned about a potential civil case - they are looking at whether there were any criminal actions or moving violations.
