    It sounds like you may have a workers compensation claim. If you haven't already, you should let your supervisor know what happened so s/he can complete an incident report. You should also try to figure out whether there is a security camera or surveillance of the area where this happened, and make sure the video gets saved. Unless you believe some third party was responsible for what happened, you will likely be limited to filing a workers comp claim.

    Have you already had the dental work done? If you end up settling with the insurance company, make sure it takes into account ALL the expenses you have had or may have if you haven't had all the dental work done at this point. Once you settle the case, you won't be able to re-open it. You are likely entitled to several types of damages including past medical expenses, future medical expenses (the crown may not last forever), past pain suffering and disfigurement, future pain suffering and disfigurement, and past loss of earning capacity. You may also have out of pocket expenses for things not covered by insurance.

    In Wisconsin the dog can be put down especially after multiple bites. The best thing to do is maintain communication with the local law enforcement office to be clear on the rules in your area.

    You should have a local personal injury attorney take a look at the accident report. Remember that the police are not generally concerned about a potential civil case - they are looking at whether there were any criminal actions or moving violations. Just because the police report describes the events a certain way, does not mean that is how it happened. Find out if there were any witnesses to the accident, and what they have to say. You may also be able to bring a small claims action in a case like this.

    You should try to negotiate with the insurance company one more time. Make sure they know about your hospital visit, and know how much your bills were.
