Making Homes More Affordable Program
To be considered for the above named plan the following items are needed:
o Information about the monthly gross income (before taxes) of your household, including recent pay stubs if you receive them or documentation of income you receive from other sources. If you are self employed then the last 2 quarterly profit and loss reports can be used.
o Your last two income tax return.
o Information about your savings and other assets
o Information about your first mortgage, such as your monthly mortgage statement.
o Information about any second mortgage or home equity line of credit on the house.
o Account balances and minimum monthly payments due on all of your credit cards.

o Account balances and monthly payments on all your other debts such as student loans and car loans.
o A letter describing any circumstances that caused your income to be reduced or expenses to be increased (job loss, divorce, illness, etc.) if applicable
o Two (2) most recent pay stubs (two for each borrower). If paid weekly, please provide four (4) most recent paystubs (four for each borrower).
o Length of service with current employer
o Most recent filed federal tax return (signed with all pages and schedules). If most recent return is not for the most recent year, please include a signed 4506-T.
o Proof of occupancy - a recent utility bill in your name at property address. (Please note that this, along with your credit report, will be used to validate proof of occupancy.)
o Copies of any leases for any room in your primary resident that you are renting.
