If a person wants to file on his/her own, they should follow the following link to the Oregon Court created forms here: http://courts.oregon.gov/OJD/OSCA/cpsd/courtimprovement/familylaw/flPacket4.page?
When to FileThe prior order continues to be in effect until it is modified by the court. A party should file for a modification as soon as they can substantiate that there has been a change in circumstances.
