South Carolina Living Will Free Form

The State of South Carolina provides a free form for a Living Will or its proper name, a Declaration For A Desire Of Natural Death. A Living Will allows you to make decisions for yourself during the dying process. If you are determined by doctors to be in a terminal condition (dying) or in a persistent vegetative state (brain dead) you can direct doctors to remove treatment or give you the maximum treatment. This form allows you to speak for yourself ahead of time and can take away a difficult decision from family members at a stressful time. It is recommended that you have the assistance of an attorney or experienced healthcare personnel help you fill out this form. Make sure it has that it is properly witnessed and notarized to be valid. Also make sure that it is on file with your doctor and hospital.
