The State of South Carolina provides a free form for a Healthcare Power Of Attorney. A Healthcare Power Of Attorney allows one, a principal, to appoint someone else, an agent, to make healthcare decisions for them when they cannot speak for themselves. If you become incapacitated the person you designate as your healthcare agent makes day to day decisions regarding your health.
