Have an interlock device installed in every car that is registered in your name and intend to drive.
Call and schedule an appointment with the following Interlock Providers. Dreager 1-800-332-6858; Guardian 1-800-499-0994; National 1-800-475-5490; Smart Start 1-800-880-3394. If your BAC is greater than 0.17 then you must maintain the interlock device for at least 2 years.
If you do not own a car to install an interlock device then you will need to contact your Colorado DMV and complete an Affidavit of Non-Ownership. You will still be restricted to only driving motor vehicles that have the Interlock device already installed in them. In other words, it will be illegal for you to drive a car without an interlock device.
Get a NOTARIZED Restricted License Ignition Interlock Agreement Affidavit and include a signed copy of each interlock lease agreement and each installation certificate to send to the Colorado DMV.
Purchase SR-22 insurance
SR-22 insurance is separate from your previously held car insurance. Basically SR-22 insurance is not really "insurance" but rather a certificate or bond stating that your auto insurance company is accepting additional liability and risk that you have brought upon yourself. You must maintain the SR-22 for a minimum of 3 years in Colorado.

Enroll in Level II Alcohol Education
Level II Education is a state-certified class which meets court requirements for DUI offenses. This 12-week class increases attendees' awareness about the negative effects of substance abuse. DUI classes meet once a week for two hours. Discussions focus on addressing the risks and problems related to the use of drugs and alcohol, how to best mitigate those risks, and the difference between addiction and abuse. Attendees participate in comprehensive discussions about the medical aspects of drugs and alcohol, legal implications of usage, signs of addiction, social impacts of drug and alcohol abuse on family and friends, and other topics.
Have your provider complete the DMV form stating that you have ENROLLED in their program.
Send all of your documents to the Colorado DMV
Complete the Application for Reinstatement form, pay the DMV $95, and include the proof of SR-22, the Notarized Interlock Affidavit and the proof of Level II enrollment.
You may either mail all of these documents to the DMV (COLORADO DEPARTMENT OF REVENUE, DRIVER CONTROL REINSTATEMENT, P.O. BOX 173345, DENVER, CO 80217-3345) or you may take them in person to 1881 Pierce Street, Entrance A, Lakewood, CO 80214.
At least 20 days after your documents are received by the DMV they will mail you a reinstatement letter which will allow you to get your Colorado Drivers License.
