1. Did you file for Social Security disability?
a. If not, you must file.
b. If so, you need proof of filing.
Determine your Medical Conditions:a. Need to show that: you suffer from a "medical condition, resulting in a deficiency in performance, conduct, or attendance or, if there is no such deficiency, the disabling medical condition must be incompatible with either useful and efficient service or retention in the position;
b. Gather Medical Documentation for Each Condition and provide it to us in chronological order if possible.
i. Go back as far as you are able when gathering documentation.
ii. Make sure you have a solid diagnosis from a doctor, psychologist or psychiatrist ideally you will acquire a new document from your practitioner.
c. Acquire a new medical diagnosis and letter from your medical practitioner.
