Identification InformationYou will need each parents' names and addresses, as well as the child's name, date of birth and social security number.
Paternity AffidavitIf you established paternity by way of a paternity affidavit, you will need to present this to the court. The paternity affidavit can be obtained from your local health department.
DNA TestIf paternity was not established by way of a paternity affidavit, the child and father can submit to a DNA test. If the parties do not voluntarily submit to testing, this can and will be ordered by the court.
