  • Accident location (address or relative location)
  • Date and time of the incident
  • Name and address of any ambulance, emergency room, urgent care, or physician you visited
  • Name and contact number of enforcement officer(s) that responded to your accident and wrote a report
  • Name and insurance details for all people involved in the accident
  • Name of each healthcare physician you’ve visited to treat your injuries
  • Any time away from work as a result of your accident
  • Names and contact information for any witnesses
  • Names and contact information of any insurance adjusters you’ve spoken to regarding your claim

Important Documents:

  • The declaration page of your auto policy and/or your homeowner’s or renter’s insurance
  • Photos of your detectable injuries
  • Health insurance card or policy details
  • All correspondence with the insurance company
  • Photos of your property damage related to the incident
  • All medical billing and records from the treatment of your injuries
  • All correspondence with defendant or any other involved parties
  • Any other receipts for out of pocket costs related to the accident
