I have 3 kids all have the same mother and 2 are in perry county and one in morrow county and I would like to move them all to perry county and combine

The Ohio Supreme Court has a lot of forms online, I've provided the link. Reading between the lines it looks like you currently do not have custody and are trying to get custody of at least one child AND are trying to modify child support for all children. Realistically, you are going to need to hire an attorney if you are to have a chance of succeeding in accomplishing these tasks.
