I would hire a real estate attorney to draft one for you so that it is in proper form, contains the required information for your lender, and is done properly. This should not take an experienced real estate attorney more than 10 minutes if the attorney has all of the needed information. You will need to give the attorney copies of the two liens so that they can properly identify the liens in the Affidavit. I would estimate that you would pay no more than $100 to have an attorney draft this for you. If you are quoted higher, then you should check elsewhere.
