In order for DOL to put an infraction on your driving record there must first be a finding of committed by a court. There would be no record of a warning other than possibly in that officer's notebook.
If you were just given a warning then it will not appear on your record; however, it appears from your question that he also gave you a speeding ticket. This will go on your record if you do nothing with it, please mark box #3 and contest the matter and then you can decide how you wish to proceed. Often times officers will give warnings for equipment violations.
