I'm trying to figure out what forms I need to request a Child support modification at the California court office. I currently have an open case with Child support office and they are enforcing my ex-husband to make payments of $600 for 3 kids directly to them. I'm confuses on what forms I need to fill out to go to court and file the forms myself and I also have unpaid medical expenses that he still hasn't paid me back for. So what forms do I need for that as well.
You will need an FL-300 (Request for Order) and an FL-150 (Income and Expense Declaration). In your FL-300, you want to request support on top of the unpaid medical expenses. You can then add the proof of medical expenses as exhibits to your Request. You will also need your last two months of pay stubs to attach to your FL-150. I would suggest contact an attorney to simply review the documents and ensure everything is filled out correctly before filing.
