If I use MC-030, how many pages can be attached? Do I need a page saying "Attachment 1" ?
Also do I use any kind of form to file "Settlement Conference Statement"? I'm also filing MC-040 to change address. I'm wondering what should I fill as Judicial Officer and Department in MC040. Thank you.
To properly file this document, you should get a declaration from your FC 4331 expert indicating his skills, knowledge, education and experience (attach his cv or resume as an attachment), then attach the vocational report with a brief summary of the facts of your case laying out the foundational and logistical manner in which the party was evaluated. If you proceed to trial, it is very likely the expert will need to testify to introduce his findings, conclusions and data.
As for the settlement conference statement, there are numerous examples on the web. Check your local rules to see if there are mandated or required content/format prerequisites that must be met.

You can use the MC-030 but state in the MC-030 what you are attaching. There is no page limit to attachments. As to the Settlement Conference Statement" this is normally done on what is called pleading paper for which there is no form. But your local court may have a local form for this so check with the court house at their self help center. If there is no form, the self help center can show you what to do.
