Looks like you are asking how to get a court-appointed lawyer to represent you. You can fill up the form at the clerk's office and if you will qualify then a public defender will be appointed to represent you on that charge. However, the state charges the fee for that - if you convicted $120 will be added to your court costs.

When you go to your arraignment the judge will ask you if you plan to hire an attorney or need one appointed to you. If you qualify financially then the court will appoint one. If not a reasonably priced attorney will charge 1500-2500 depending on the circumstances and jurisdiction.
