Yesterday, she blocked my car in a restaurant with her car and I couldn't move for 2 hrs. The police was called and they stabilized the situation. She has sent me messages in the past threatening me. She further told me yesterday that I would die a horrible death. I have a video recording of this. I tried to get an EPO but the judge denied it. I'm frustrated and scared that she will attack me.

Is there a hearing scheduled for a protective order now that the EPO was denied? If so, get a lawyer to represent you and to send her a cease and desist letter. Call the police anytime she threatens you or appears violent or harassing. Good luck.
Get an attorney and get a restraining order. Cannot imagine why EPO was denied.
