I am planning on signing a license agreement for a new office for my company, which basically just covers your terms, what you will be liable for, what will happen if damages happen, etc., and I noticed that in the signature, it shows where I have to sign, print date, my name, and title of the company, but no where in this agreement does it have where my company name is entered. This left me concern that I will be personally liable (the agreement does have a personal guarantee section but i am waiting to hear back to see if its required to be signed as well) and I want it to be completely my business that is liable because if I retire, or if anything happens to me that renders me not to be able to perform my duties (doubt I will but you never know what will happen in the future), I dont want to be stuck with the bill if the company doesnt cover it. Should I be concern about the liability or should I be safe? On the card authorization form and contract receipt shows my name and the company name (without the title), so I feel its just a way to tie me to the bill regardless.
You mentioned that no where on the agreement your company name is entered. That certainly raises a concern. An attorney would need to review the language of the entire agreement as well as the signature page.

It appears that this contract is one which creates personal liability for you. You should take the contract to an attorney for review. There are likely other issues in the contract which you may not be aware or understand. An experienced business attorney will be able to explain.. The small fee you pay will be an excellent investment BEFORE you sign a contract.
Without reviewing a copy of the agreement, it is difficult to answer the question. It does sound, however, that the landlord is entering into the agreement with you personally, not your company. It's not uncommon for personal guarantees to be requested, however, you should really have an attorney to review and advise.
