There is a "form" in the the Florida Rules of Civil Procedure called a Form 1.977 Debtor Fact Information Sheet that has questions that a judgment debtor must fill out under penalty of perjury about their financial status, and also requires that they include certain financial document with the completed Form 1.977. The judgment holder holder can either have language requiring that the defendant (judgment debtor) complete the form (within 45 days) included in the judgment itself when the judge enters judgment, or the judgment holder can file a motion requiring the form be completed after the judgment has been entered.
