Go to your local District Court and request an anti harassment restraining order (MGL 258-E). You will have to fill out a complaint form, an affidavit explaining the basis for the complaint, and appear briefly before the judge to testify was has occurred. There is usually a victim advocate volunteer available to assist you and there is no charge for the complaint. You will have to appear again in about 10 days to extend the order for up to a year and your ex-gf will have an opportunity to defend herself, but you seem to have ample grounds – if you have any evidence to support you complaint (text messages, e-mails, witnesses, etc.) bring that with you. Once you have obtained the order it will be a criminal offense for you ex to contact you, directly or through third parties.

Yes, get the 258E anti-harassment order. For the court hearing, bring any and all written proof you have, such as e-mails and texts, or any messages left on voicemail or answering machines if they can be easily played in court. Judge may not listen to the messages but it will help your credibility to know they exist and are right there. But even on your testimony alone, you should be in a good position.
