You are over thinking it. It does not need to be fancy. Type a document entitled motion for early termination of probation. List in numerical order the terms of the plea, the length of probation, how much probation you have completed and then also the specific reasons you think you are eligible for early termination. Send a copy to the state and the judge and file the original with the clerk of courts. In the motion ask for a hearing on the matter. Follow up its the judges judicial assistant.
