Is proposed order mandatory?
I filed a motion for dismissal, without proposed order.
In some cases (on internet) the judge prepared own order form, so I thought proposed order is not mandatory.
But on a website (pro se guide for family issue) by Superior Court there is a direction which tells parties to attach proposed order.
Should I attach a proposed order? If so, I need to refile the motion.

If you are in doubt, you should call the judicial assistant for the judge assigned to your case and discuss it with them. They can let you know whether their judge will expect you to file the proposed order or not. You can find information for the judge's office on the court's website. The phone number listed for the judge will connect you to the judge's assistant. Judicial assistants are generally very helpful and knowledgeable.
