Best source is your attorney handling the H1b for you. However, as a general rule you must be qualified for the profession you are seeking. A license to practice is a must otherwise John Due can do what you are qualified to do.

You must be eligible for the job at the time of filing of the petition - so you must be licensed to practice medicine, if that is a requirement for your job, at the time the petition is filed. Your employer's immigration counsel should be able to counsel you on this issue.
