Is CPS required to notify me if there is an investigation opened and concluded against my ex about my child?

You should be able to obtain a copy of the CPS report, and I agree with you that it is sort of strange that they didn’t contact you to get your side of the story. Depending on what is stated in the report, you may want to consider filing a motion to modify custody and placement. Talking with an experienced family lawyer in advance will help determine if you have a case.
