You should consult with an attorney who can help you draft a prenuptial agreement laying out clearly what will be considered matrimonial property during the marriage.
If you are concerned, you can talk to an attorney but generally pre-marital debts remain with the person who incurred them through marriage. That being said, if your spouse has significant medical debt/other debt, be cognizant of how that will affect your finances moving forward.
