Yes, those are the correct forms. It could take up to 3-4 months for the Petition to be granted. If you need it done quicker, you need an attorney to file a written motion and get it put on calendar. If you file it yourself, don't forget to serve the District Attorney.
Those are the forms. However, you should know that this will not take anything off your record. It will just add a notation to your record that the conviction has been dismissed.
Before you have it set aside via 1203.4 of the penal code. Ask the court to reduce it to an infraction. You need to ask in writing at the same time you ask for the charge to be set aside.
