HIPAA is very strict and a medical professional should not violate the patient's privacy unless it is to prevent serious harm. It's a judgment call for the doctor, so if you gave any indication that you would not admit the event occurred or would minimize it or some assurance that is was indeed isolated, the doctor probably won't risk the violation.
And if you don't want to pursue it, you shouldn't make statements that it occurred in a public forum like this. But more, you should consider talking to a professional about why such a violent encounter, you smashing things, and him choking you so bad you needed to go to the doctor, even occurred at all. You didn't say you left, and it's over, so I assume you're still in that space. Maybe you should consider a change, as that kind of behavior is not healthy.
