I owe money on a credit card that went to collections. This subpoena is asking me to fill out a provided questionnaire only. Is tgat valid? There's no judge's name on it or court address. Only the attorney's address

It it sounds as though the debt has been sued upon and there is now a judgment. Article 52 of the CPLR specifically provides for information subpoenas. Failure to answer and information subpoena may result in an application for your incarceration as a contempt of court. Obviously this is that something that you want to play around with.
Please note that there may also be a re-straining notice included with the information subpoena. Frequently they are part of the same document. That is a very serious matter. Disobedience of a restraining notice by paying anything other than the creditor is also punishable as a contempt of court.
