I'm a Canadian working in the US on a L1 visa and I'm currently on short term disability. Can I move back to Canada?
Could the insurance company ask me to pay them back if I decide to stay in Canada once they stop paying the compensation?

Very interesting question. First, your policy may have a travel restriction. Violating that would disqualify you, if there is one in your policy. Logically, they analyze your occupation as performed in the national economy. That would still be the United States. Here is the more logical issue, if you move away, they may try to terminate/deny you just because it is now more difficult for you to fight with them.
In short, its a risky proposition. If there is no policy language covering this, then you CAN do it, but it may cause trouble. Tough position to be in, best of luck.
