Your marriage certificate and your divorce decree are two separate documents. Your decree, if you wanted restored to your maiden names, would serve as the order to do so. If not you must petition the probate court for a name change. A copy of your birth certificate and a copy of the divorce decree should suffice to show your maiden name and that you were divorced. The marriage certificate issue may be that the person solemizing the marriage never filed the acknowledgment that the ceremony was performed. The decree should resolve that discrepancy as both parties did not dispute that the marriage existed.
