They want proof you have a clean bill of health from your doctor for seizures etc...
Speak to your doctor as they should know how to write the letter needed.

It would seem that because of this sort of accident, State wants some reassurances from your doctor. Your doctor just needs to fill it out and if your health is as you report, then there will probably be no further problem.
