If only a minute order was prepared but the other party never turned in the final paperwork for the case and it's still considering "pending", does it ever expire? Can I counterfile? It's been 3 years.
You don't provide enough information to properly answer your question but if there is an order that is 3 years old, it is very likely final and there is nothing you can do to change it. You will need a specific review by an attorney to know for sure.
...and if the minute order contains an order for one of the parties to prepare an Order After Hearing, I suggest you go to the self-help center and have them assist in preparing it.
