Same as you do for a trial. The lawyer prepares the subpoena and has it served on the witness with the date, time and location of the hearing.
A plea withdrawal hearing does not usually involve testimony. The motion is decided on the moving papers (with witness affidavits) and the opposing papers. If you did follow the proper procedures and successfully subpoenaed someone to Court I am not sure the judge would be interested.
