I received an FL-374 "Notice of Appearance and Response of Employee Benefit Plan" from Attorney who represents the City Employee Benefit Plan.
In section 3b., where the claimant responds to the pleading on the joinder the FL-374 states
"Pursuant to California Family Code Section 2063(b), Claimant does not respond to the pleading on joinder. All statements of fact and requests for relief contained in the pleading are deemed controverted pursuant to said section 2063(b).
What does this mean in "laymans" terms and did I not properly fill out the forms properly for the joinder? Did I make a mistake of fact?
Can I process the forms to the family court or do I need to re-do the forms?
In laymen's terms: see an attorney
The City denies your claim
And states that in need not separately respond to the pleading.
Your situation is way too complicated for someone to respond to on a general internet question and answer site. Time to sit down with an attorney to review your situation. I am sure that is not what you want to hear but how do you reasonably expect someone on this site to determine what happened in your case?
Please post this question in the family law section of Avvo. You will get far more precise and helpful question from the attorneys in that forum who specialize in this area of the law.

All of Ms. Straus’ responses are intended as useful information, based solely upon the facts stated in the question, and are not to be relied upon as a full or complete legal opinion. They may not be what you wished to hear, they do not create an attorney-client relationship. Ms. Straus has been licensed to practice law in California for 33 years. Ms. Straus regrets that she does not provide follow up free advice via email. She also regrets that blunt responses may be taken as "rude" by those who wished a different opinion. Good luck.
