You need to go to the Common Pleas Court and file a Nunc Pro Tunc petition. Some courts require you to file the summary appeal at the same time. In any event most Clerks will have a form for filing if not you should be able to pull one off of AOPC. As a practical matter, the burden for allowance of an appeal Nunc Pro Tunc is pretty high. You should consult with local counsel because they can help walk you through the process and increase your likelihood of success. Some prosecutors will allow the Nunc of they can reach an agreement on the Summary Appeal. Local counsel will be in the best position to advise you on this.
