I lost my annual guardian ship papers before filling out and returning can I get replacement paperwork
I strongly suggest you contact the court which granted you guardianship over your ward. An alternative is if you have the information here is a link to a blank form that may be the form you are talking about.

The annual report is pc634, Annual Report of Guardian on Condition of Legally Incapacitated Individual
If you need to file an accounting too, the forms are pc583, Account of Fiduciary, Short Form, or pc584, Account of Fiduciary, Long Form, depending on the one the court requires you to use.
