How can I be charged with no arrest and no citation?
Recently I received my SF-86 investigation report and in the court records saw SDPD filed charges against me and then rejected the charges.
I have a letter from City Attorney states "a complaint has not been filed. At this time, we do not intend to issue charges."
In the SF 86 I stated I never charged. Was I charged or not?

It is less common that you were not arrested in a DV case. SDPD cannot file charges. They submit them to the prosecution, who either files or does not file the case. From what you are saying it sounds like charges not filed.
If it says charges filed on your records, you should send corrections to the DOJ.
