Work comp won't pay for food for my overnight trips. They also owe me money backdating a year for mileage. They make me drive over 200 miles for a doctor appointment each month. Everytime I talk to my attorney he says that if I wanted to settle it would be lumped in but since I am not settling and want to keep my future medical, I am out of luck and have to eat it. I don't know what to do

Meanwhile, to answer your question, they should be paying for mileage. You would still have to eat if you were at home so there's no requirement that they pay meals. But...if your physician says you can't drive the 400 miles a day and have to stay over, they would then have to pay meals & hotel expenses.
If your doctor writes that you can’t travel 200 miles in a day, then your attorney will have an argument that they need to pay for an overnight stay and meals as well as mileage. I know medical care is limited in Eureka but you have the right to change physicians to someone closer to your residence.
Good questions for your attorney. Generally, reasonable medical mileage is reimbursable. If they have authorized your doctor appointment, it would be a little unusual to pay you nothing for over a year for mileage. You might be able to file for a court hearing on the issue, as Im sure it has added up to thousands of dollars in reimbursement.
