SC-140 is the Notice of Appeal which you would only file if you appeared at trial and lost the small claims court case.
SC-135 is used if you already missed the hearing (trial) date altogether.
If you wish to transfer the case to your home county, you can use SC-105 for a motion (or simply just write a letter to the court, referencing your trial date and the case number) stating the reasons why the case ought to be transferred to your county.
