Do I need to remove deceased dad's name from r.e. deed/county records before transferring deed to mom's trust?

If the date was in both their names you have to file an affidavit of death of joint tenant and a copy of the death certificate. That will leave the deed in her name and she will then be able to transfer the property into the trust with a trust transfer deed. If the title is some other form, that will change the process. No, you cannot do it in one transaction.
