A name change can be done very easily by including it in the Decree of Divorce. Unfortunately, if the name change was not included in the Decree, you need to file a petition for a name change.
Another possibility (and I don't guarantee it will work), if your ex spouse would agree to it, would be to stipulate to an amended Decree of Divorce that includes the name change provision.
You should consult with an experienced family law attorney to make sure the paperwork is done correctly in any event.
