I am working with Hospital A now( H1B visa expires 2020), got better offerer and bigger place Hospital B, have offer letter from them. Informed hospital A, they are ok with me leaving the place and going to hospital B.
my question is what is best way to change the job, when should i resign here, or Do i have to resign or not. can I give 3 months notice, does that work.

Hospital B does not need Hospital A's consent to file an H1B transfer petition.
You can work for Hospital B upon filing under H1B portability rules. However, this is risky if the H1B petition is denied. As for timing the departure from Hospital A to B (normally employment is at-will unless you have signed a restrictive Employment Agreement), this can have ramification on grace period or can be a violation of status if prematurely starting employment with Hospital B.
