Medical malpractice cases in Texas are tough these days to find a lawyer to take them due to all of the tort reform passed in Texas which capped the amount of damages one can recover and also made the cases more difficult to prove and win. There is generally a 2 year statute of limitations on this type of case. What effect the paper you signed has on the case is almost impossible to answer without reading the paper. You may be able to argue that the paper is void because you signed it under duress. The big issue I see is whether the doctor committed malpractice or not. Doctors usually assert that there are risks to every procedure. Did you sign a consent form prior to surgery which detailed the risks? You will need to hire a lawyer who will handle this case and the lawyer will then have to hire and pay an expert witness to review the case to determine if there was negligence/malpractice and whether that caused injury, and then you can proceed with suit. Almost all family law cases go to mediation and it is not a sign of weakness for an attorney, whether practicing in family law, personal injury law, or any type of law, to participate with his client in mediation. Most courts now require all civil cases to be mediated prior to trial.

I am an attorney and I practice law in Texas, and I am a professional mediator. If the mediation is scheduled by the parties and/or attorneys of the parties, and it was ordered by the Court that the parties attend mediation, not showing up once would be enough to file a Motion for Contempt, or Motion for Sanctions.
It is contempt to violate a Court ordered mediation. it is sanctionable.

To try to get reimbursed for sub-standard dental work (i.e. a dental malpractice claim), I strongly feel that your most likely way will be to contact a local medical malpractice attorney who represents Plaintiffs. You need legal representation to present your claim properly.
Also, I do not practice law in Oregon, but I am sure there is a statute of limitations with respect to your claim. in Texas, you must file suit within two years of the malpractice. So you better call an attorney ASAP.

I would recommend that you contact a personal injury attorney in your local area. He or she may be able to help you with this adjuster and/or help you get medical treatment now (maybe with a letter of protection saying you will pay the dentist upon settlement of your case) and then settle your case with the insurer.

Your property damage should either be paid by your insurance carrier or the other driver's carrier. You did not mention any injuries, so I assume there are none. If someone was injured, that person needs to hire an attorney to pursue the injury claim. But as stated before, the carriers will probably arbitrate the matter to determine which the pays the property damage.See question

No offense to gym owners, but I have always been leary of joining gyms for this type of reason, and I had a circumstance one time where I had trouble getting the gym to stop charging my bank account after I canceled membership.
You can certainly write a letter to the gym and collection agency and advise them of the proof that you canceled in writing on such and such date (hopefully this was allowed in your contract to cancel early) and to advise them to refrain from any action to damage your credit, or you may be forced to take legal action.
Or if your contract did not allow you to terminate without owing these gym fees, you can pay the $700.00. Or, if your contract allowed termination, you may choose to not pay and dispute anything that goes on your credit or a lawsuit if one is filed against you.
The problem with contacting or hiring a lawyer is that the dispute is over a rather small amount, so I am sure you do not want to pay an attorney over a now $700 dispute.
